We never find out, but I believe Crim said that it was some sort of accident, dialogue by Morganna later in the series seems to verify this.

Hmmm, I think the timing of her accident would also put it in line with the Pluto's Kiss virus, perhaps it was something related to that?

Probably. The Tsukasa clone says something about "unexpected car accidents" to Subaru, so that's what might have caused it. Perhaps a traffic signal connected to the internet malfunctioned?

Anyway .hack does make a pretty big point about how dangerous having everything connected to the internet can be. Particulary if it's all using the same type of programming.

Breaking a leg wouldn't put you in a wheelchair for the rest of your life... it was a much more serious injury then that.

Actually, I've heard that it was she was hurt during the Pluto's Kiss incident, but that's probably speculation also. Not sure on the source of that though, but it's possible. Maybe the accident paralyzed her, rather than just breaking her legs? That's a possibility also.

That can be found in .hack//Encyclo, too. But it's stated there that it's only a speculation. But I if I'm not mistaken, it's stated (in zero or analysis, I forgot) that Subaru wasn't born with ther legs disabled

In SIGN, episode 23 I think, Crim mentions to Tsukasa, "Subaru hasn't been able to walk since four years ago", or something along those lines.

So no, she wasn't born disabled.
