I'm positive that's the real answer, but it's so boring! Let's find something improbable and creative! Maybe the entire show is supposed to be about solipsism, the idea that you are the only person who exists and everyone else is "less real." Perhaps Tsukasa is the only live player in the World ... perhaps it's all an illusion created by Morganna. She only has so much processing power, and thus can't create too many players cluttering the background! Ta-Da!

Where are all the people?
You see root towns 3 - 4 times in the series. They mainly are seen in abandoned areas, places the averge person wouldn't visit, unusual dungeons. These aren't you're avergae locations. For the amount of people playing, their are probably hundreds of dungeons, fields, root towns, structures, etc. 
What I do find surprising is nobody is ever at the gates, even larger more common ones, which is highly suprising, it being your average starting point.

Well, you aren't gonna hang out at the Chaos Gates. You're gonna go to a field or something. And yes, a lot of the show was in empty fields, abandoned ones where they could go to be alone. As for people, in some scenes they do show a large number of people, like in the first scenes of Mac Anu or C. Gadelika.

I agree, it would cost them a lot of money to animate many people walking around the world. In the first couple of episodes there is a lot of people walking around, but then all on the sudden they all disapere.But I'll make a stupid answer again. Maybe the animators didn't wanna animate more people ^_^
Maybe with a movie budget, they could.
