Note that I haven't seen all of Sign yet, nor any of Liminality besides what's on the demo disc, but why don't you ever see Shugo or Rena's faces (up to ep 6)? We've seen a nose and a mouth, but that's it. It's rather creepy. Especially the shot of Rena towards the end. Is it a symbolism thing or what?
That's a continuous trend in all of the .hack series, except for the ending of one instance in the series and in Liminality as a whole. However, whenever they're playing the game in Liminality 1, they never show you what Mai and Toku are playing.

Yes. They do that to keep the separation between the game world and the real world. In //SIGN, you could never see anyone's entire face, the eyes were always hidden. Likewise in //Udeden. Liminality, as it takes place in the Real World primarily, doesn't let you see the character when they play, like Tsukasa said.
It's mostly symbolic, but also helps keep down confusion as to whom is whom.
