They didn't know that the game was to blame for all the problems. Sure, people had their suspicions, but all the doctors were saying that it was impossible for the game to cause comas. Also, CC2 didn't want to lose any money. 20 million people play the World. If the game gets banned, all the profit goes down the drain. Nobody could prove that the World itself caused the trouble, so it stayed online.
Yes, but almost the entire world switched to the Altimit OS after Pluto's Kiss. Altimit is a perfect system. It's completely error-free, and viruses like the one that caused Pluto's Kiss have no effect on it.

Don't you think some people thought that Tsukasa could have been lying about not being able to log out? I mean, 20 million people is a lot of people for no one to have suspicions

Watch .hack//SIGN episode 28 "UNISON" and Subaru talking Mimiru cleraly state that Bear is the legal guardian of Tsukasa and then later Bear comes on and says its though being a parent again or something like that, I'll post up screen caps later

I honestly don't think everyone knew that Tsukasa couldn't log out. The first person Tsukasa told was Mimiru, which slowly made it's way around the main characters. However, that was where it stopped. If rumors did spread out, then Tsukasa would be bombarded by emails.
